Regular appointments are a necessary part of a treatment plan.
Contact the office by phone if you need to cancel or change your appointment within 72 hours of your scheduled time.
At this time, all appointments are scheduled by calling the office.

Reminder emails or calls are made to ensure patients can keep their scheduled appointment.
Call the office to verify your appointment if there is inclement weather in the area or to request your appointment be changed to a telemedicine visit.

At the doctor's direction, current patients may be directed to download, print, and complete the following forms or rating scales for their appointment.

Twenty four hour notice is required to cancel or reschedule an appointment and avoid a "no-show" fee.

Credit Card payments can be made at the time of your appointment.
Account balance payments can also be made by credit card or a check can be mailed to the office.
